I am a
doctoral student in the Doctor of Health Administration program in the School of Health Related Professions at the University of Mississippi Medical Center. I am conducting a research study to explore the experiences and perceptions
of a select group of health care employees in Mississippi regarding comprehensive eye exams as part of regular preventive health services.
You are being invited to participate in this study because you work in a health care organization.
If you agree to participate, you will take a brief survey that should take no longer than 15 minutes to complete. Your participation in this study is
voluntary. If you choose not to participate or to withdraw from the study at any time, there will be no penalty and you will not be affected in any way. The questionnaire is anonymous. The results of the study may be published,
but your name will not be known.
